For more information on your pregnancy you might like to download a free app - either Baby Buddy or Pocket Midwife are free and offer lots of helpful advice. Alternatively you can click on the link below to view NHS Choices information on Pregnancy and Birth.
We are a Yellow Fever Centre and can also offer Yellow Fever Vaccinations to patients who are not registered at our surgery.
Travel Health Questionnaire
To help us offer the appropriate advice, please fill out the travel form, either click on the link below or pick one up at reception and return the completed form to us at least 5 weeks before your departure date.
Information for travellers and questionnaire
If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.
There is further information about countries and vaccinations required on the links below
It is important to complete the travel form at least 5 weeks before you travel - as an appointment will be required with the practice nurse to actually receive the vaccinations at least two weeks before your departure date. These vaccines have to be ordered as they are not a stock vaccine.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
Travelling in Europe
If you are travelling to Europe a very useful booklet has been published with advice and guidance to help you get the most out of your holiday. To visit please click:- http://ec.europa.eu/publications/booklets/eu_glance/86/en.pdf (this is a large document and may take a minute or two to view)
Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:
The fees charged are based on the British Medical Association (BMA) suggested scales. Please contact reception to enquire about current Fees & Charges
You can download information about the service provided by our Health Visitors here.
If your GP has made a referral for you please click the following link to print off the patient referral leaflet to take to your appointment. This leaflet describes what you can expect to happen when your GP refers you to see a specialist or consultant at a hospital or a community health centre. Please read through and make use of the checklist to ensure that you have everything needed whilst at your clinic appointment (for example medication/fit notes).
