New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. If you don't have a medical card you will need to complete a registration form. We also ask you to complete our health questionnaire and 'Sharing your GP record' which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be downloaded (please see below). If you are aged between 40 and 74 years, you are entitled to an NHS Heartcheck with a member of the healthcare team. If you are eligible for a check, please book an appointment at reception.
If you wish to register please bring the completed Registration Form, New Patient Health Questionnaire and Sharing your GP record form to our reception team.
If you require information or communication in an alternative format (eg. large print, braille) please indicate this on your Health Questionnaire. We will then make reasonable adjustments to accommodate your needs. In addition, the following website has some useful information- including auditory and visual aids.
Medical treatment is available from the date of registration. Please contact reception for further information.
